Open the catalog record for the product and strength you need. Starting from the product page reduces ambiguity because the request link can carry the selected product identifier into the documentation form.
Before submitting, confirm the full product name, labeled strength, and the email address that should receive follow-up.
Use the lot identifier when available
A lot identifier ties a documentation request to a specific physical vial or associated order record. Enter it exactly as shown, including letters, numbers, or separators. Avoid substituting an order number when the form asks for a lot.
Product name identifies the catalog record. Strength identifies the selected variant. Lot identifies the batch-related record connected to the vial.
Add context without changing the request
Use the notes field for a concise clarification, such as the associated order reference or the reason a label is difficult to read. Do not place sensitive personal, medical, diagnostic, or treatment information in the request.
Submitting the form creates a documentation request. It does not reserve stock, place an order, or change the product’s public availability.
Review the returned record against the vial
